Please note this form is for gym, swim and class memberships, for swimming lessons, please fill out the swim lesson cancellation form or email hampton.pool@southdownsleisure.co.uk Please note, due to a high volume of membership changes we are not always able to confirm once your request has been actioned, but as long as you have provided us with 30 days notice and selected the appropriate date relating to your Direct debit, then the cancellation will be actioned

Please base this on the day in which payments come out of your account. Which should be the 1st, 8th, 15th or 22nd. Whichever day you select will be the day we stop payment going out from. We need minimum 30 days notice (from the day the collection is due from your account) to action any cancellations.
